Overview
Forms will no longer be supported on data platforms, and will be deleted from all data platforms in January 2025. An embedded Google or Microsoft form within Perspectives Stories is the replacement for legacy platform forms.
Recreate With Microsoft Forms
Step 1: Create The Form
Follow these instructions to create the Microsoft form you will be embedding into a Perspectives Story.
Step 2: Automate Form Response To Platform
If you’re using a Microsoft form, you will need to use Power Automate to automatically populate responses into an Excel spreadsheet. Refer to this article on how to setup Power Automate: Common ways to use a form in a flow - Power Automate. Storing this Excel workbook to a mapped network drive, or syncing the Sharepoint Folder to a mapped network drive will allow you to use the Excel plugin create a dataset and automate updates to the platform with a schedule.
Once this automation is set up, you will need to set up a Gateway using the Excel Plugin to pull your responses as a dataset.

Step 3: Generate Form Embed Code
To generate the form embed code from a Microsoft form follow the steps below:
- Open the Microsoft Form you want to embed
- Click the Collect Responses button, which is usually located at the top right of the form editor
- Select the appropriate permissions for your form. Only public forms can be embedded into a Perspective Story page. The audience of the form can then be managed in the data platform when publishing the Perspectives Story.
- Select the Embed option, which looks like two brackets (</>)
- Copy the embed code that is provided
Step 4: Embed Form In A Perspectives Story Page
In order to embed a form into a Perspectives Story page, you must have a Perspectives Story open in edit mode. For guidance on how to create a new Perspectives Story, see here.
Once you have the Perspectives Story open in edit mode:
- Add a content block to the Perspectives Story via the right hand menu to the location you wish your form to display.
- Click the “+” icon in the content block
- Select “Other Media”
- Select “HTML Embed”
Once you are in the HTML embed pane, copy the embed code generated above in Step 3 into the “Paste or type HTML code” text field. A preview of your form will generate and display below. You can now “insert” the form into the Perspectives Story, and publish the Story to make the form available to any users who have access to the Perspectives Story.
Recreate With Google Forms
Step 1: Create The Form

Step 2: Automate Form Response To Platform
If you’re using a Google form, you will need to then connect the data using the Google Sheets plugin via a Gateway.

Step 3: Generate Form Embed Code
To generate the form embed code from a Google form follow the steps below:
- Open the Google Form
- Click Send in the top right corner
- Click the Embed option, which looks like two brackets (< >)
- Copy the HTML code that appears
Step 4: Embed Form In A Perspectives Page
In order to embed a form into a Perspectives Story page, you must have a Perspectives Story open in edit mode. For guidance on how to create a new Perspectives Story, see here.
Once you have the Perspectives Story open in edit mode:
- Add a content block to the Perspectives Story via the right hand menu to the location you wish your form to display.
- Click the “+” icon in the content block
- Select “Other Media”
- Select “HTML Embed”
Once you are in the HTML embed pane, copy the embed code generated above in Step 4 into the “Paste or type HTML code” text field. A preview of your form will generate and display below. Additional Details
- Only public forms can be embedded into a Perspectives Story. Access to the form is then managed on the data platform by selecting the appropriate publishing audience when publishing the Perspectives Story page.
- Changes to a form that is already writing to a dataset will result in an error for subsequent updates. Instead, if a form needs to be updated in way that changes the schema of the dataset it is writing to, users will need to follow the steps above to map it to a new dataset.
- Set a schedule on your dataset to ensure data is up to date. See this article for instructions on setting up a schedule.
- Embedding HTML content works best in classic mode Perspectives Story pages and will experience trouble resizing in Perspectives Stories using Flexible Layouts.
